The End-of-Life Pathway provides $25,686.28 over a 12-week period, accessible for up to 16 weeks, for participants nearing the end of life who wish to remain at home. It is accessed instead of your ongoing quarterly budget for that period.
What the End-of-Life Pathway is for
The End-of-Life Pathway provides a higher, dedicated level of funding for participants assessed as having approximately three months or less to live who wish to remain in their own home rather than move into residential care. Because care needs typically intensify sharply in this period, the pathway replaces the ongoing quarterly budget rather than sitting alongside it — it is designed to fund a short period of significantly higher support, not to be combined with the everyday-services budget at the same time.
